Feedback Form
We are continually striving to build an organization that is responsive to the communities we work with. Recognizing we can always improve our operations and always need to be responsive to concerns as they arise, this form is a place for you to send us any feedback you may have.
This can include:
- letting us know what is going well
- informing us about concerning behavior by our drivers
- any problems with our accessing our services
- or anything at all that might help us improve our services.
The submission on this form will be reviewed at our next drivers’ meeting. These meetings take place approximately weekly. We have included a space for you to let us know if there is anyone in particular you would like to handle your feedback, as well as if there is anyone you do not want to handle this feedback.
If you request a response from us, we will try to respond within two weeks. Please let us know your preferred method of contact in the form below.
